Critical Diabetic Patient Saved by TRIHMS’s Cardiology team in State’s First Primary Angioplasty

TRIHMS’ cardiology team achieved a remarkable feat – the state’s first successful primary angioplasty – saving a 61-year-old diabetic man with critical health issues.

The 61-year-old man had come to Arunachal to visit his ITBP son. He developed chest pain in the evening and was rushed to RKM, where he was diagnosed with inferior wall myocardial infarction (heart attack) and further referred to TRIHMS.

As he was in the window period, he was immediately thrombolysed with STK (Streptokinase). Despite this, he developed cardiogenic shock. He was then immediately shifted to the cath lab for primary PTCA (Percutaneous Trans Coronary Angioplasty).

A temporary pacemaker (TPI) was first inserted, followed by CAG of the left coronary artery (LCA), which showed significant disease in the left anterior descending artery (LAD).

Then, before the right coronary artery (RCA) could be engaged, he developed VT followed by cardiac arrest.

The patient was then given a defibrillator shock, and CPR was started. Simultaneously, the RCA was hooked, and CAG was done, which showed near total occlusion of the culprit proximal RCA.

Ballooning followed by stenting of the proximal RCA was performed. Before stenting, he developed VT again, for which a defibrillator shock was repeated. Once stenting was completed, his vitals began to stabilize, and the VT gradually disappeared.

He was then shifted to ICCU for monitoring and recuperation. The cardiac interventional procedure started at 12 am and completed at 1 am of June 2.

The patient was very critical and suffered two episodes of cardiac arrest during the angioplasty. If not for the emergency cath lab facilities with a 24/7 specialized cardiology team, the patient would not have survived.

The cardiology team that successfully performed the angioplasty included Dr. Rinchin Dorjee Megeji (Associate Professor, Cardiology), Dr. Tony Ete (Assistant Professor, Cardiology), Dr. Sindu Damde (Junior Resident, Cardiology), Margo Bagra (Cath Lab Technician), Mukhtarul Ali (Cath Lab Technician), and nursing officers Karnu Poyom and Sandhya.
